WHEREAS, the Client needs to hire the services of the Agency to provide Janitorial Services for ________________________, hereinafter referred to as the “Premises”, located at ____________________________________;

WHEREAS, the Agency is duly licensed to engage in the business of providing the services required by the Client and has offered to render its services to the latter; a copy of DOLE Certification is attached hereto as “Annex A,”;

WHEREAS, the Client agreed to accept the offer of the Agency, subject to reasonable terms and conditions; 

NOW, THEREFORE, for and in consideration of the terms and conditions freely and voluntarily agreed upon by the parties hereto, it is agreed that:

1.  SERVICES   - Agency undertakes to render and perform cleaning services in Premises. 

It shall efficiently perform the services enumerated under “Annex B,” which is hereto attached and made an integral part hereof. 

2. CONSIDERATION – For and in consideration of the services to be performed by Agency under this Agreement, Client shall pay Agency the rates specified in “Annex C,” which is hereto attached and made an integral part hereof, payable monthly, within ______ (__) days after submission by Agency of a duly signed Statement of Account and after verification of Daily Time Records.
